Good Hearts LLC was founded in 2026 by Grace Carolinda, with a commitment to bringing dignity, warmth, and steady care to families across Colorado.
For too long, families navigating the world of intellectual and developmental disability services have been told to settle. Settle for a placement that doesn't fit. Settle for care that feels transactional. Settle for waiting and worrying.
Good Hearts was started to push back against that. We believe care should feel like home. We believe providers should be supported, not just deployed. We believe families should be heard, not just processed.
We are a small, locally rooted agency. That's intentional. We choose to know every individual we serve, every family we work with, and every provider on our team. When something matters to you, it matters to us.
Good Hearts works within Colorado's HCBS waiver system, partnering with multiple Case Management Agencies and Community Centered Boards across the Front Range. Our four service lines (IRSS / Host Home, Supported Community Connections, Community Connector, and Youth Day Services) are built to align with the HCBS-DD, HCBS-SLS, and HCBS-CES waivers.
Every plan starts with the person, not the paperwork. We ask first, decide together, and adjust as life changes.
We are Colorado-based and Front Range-focused. We know the system, the CMAs, the CCBs, and the community we serve.
