HCBS-DD Waiver

Individual Residential Services and Supports

IRSS / Host Home (DD)

Through IRSS, a trained Host Home Provider opens their home as a long-term residence for an adult with intellectual or developmental disabilities. It is not a facility. It is a family setting where the person belongs, contributes, and grows.

Good Hearts carefully matches individuals with providers who fit their personality, routines, and goals. We provide ongoing oversight, training, and support so both the person and the provider thrive.

  • 24-hour shared living arrangement in a state-approved provider home
  • Personalized service plan aligned with the individual's HCBS-DD waiver
  • Ongoing case coordination with your CMA / CCB
  • Provider training, support, and emergency backup
  • Regular wellness checks and monthly home visits

Supported Community Connections

SCC (DD/SLS) ยท Day Habilitation

Supported Community Connections is structured day habilitation programming designed around each person's interests and goals. Activities range from community outings and volunteer work to skill-building, art, fitness, and quiet time.

Every day looks different. Every day has purpose. We build routines that respect both individuality and the value of consistency.

HCBS-SLS & HCBS-CES Waivers

Community Connector

CES Waiver

Through the Children's Extensive Support (CES) waiver and Supported Living Services (SLS), our Community Connectors work one-on-one with individuals to help them build relationships, access resources, and engage in community life on their own terms.

This is not a program. It is a partnership. A Community Connector becomes someone who helps the person navigate the world, advocate for themselves, and discover what they love.

  • One-on-one community engagement and skill-building
  • Help accessing recreation, employment, and social opportunities
  • Support navigating healthcare, transportation, and resources
  • Goal setting and personal advocacy
  • Flexible scheduling that fits the person's life

For Children & Youth

Youth Day Services

After-school & weekday programming

Youth Day Services provides after-school and weekday programming for young people with developmental and intellectual disabilities. We focus on growth, friendship, and skill-building in an environment that feels safe, fun, and welcoming.

Families need a place where their child is genuinely cared for, not just supervised. Our Youth Day Services are built to be that place.
